SUMMARY:

The Intake/Investigator will receive and process all Social Services referrals including allegations of abuse, neglect or exploitation of children, adults with disabilities and the elderly, and determine eligibility for Adult Protective Services, Children and Family Services, Child Support Services, Temporary Assistance for Needy Families, Tribal Victims Services and Vocational Rehabilitation. The Intake/Investigator is authorized to investigate and respond to allegations that children are in need of protection and bring children into the care of the Prairie Band Potawatomi Nation Child and Family Services when necessary in order to ensure child safety and well-being. The Intake/Investigator is authorized to investigate and respond to allegations that adults with disabilities and the elderly are in need of protection from abuse, neglect or exploitation and to determine if there is a need of any protective and/or social services.

ESSENTIAL DUTIES:

  • Receives and coordinates all referrals
  • Responsible for the review of daily logs and notes from Case Managers to assure continuity of care, documentation of nursing process, appropriateness of intervention/action and timeliness of documentation.
  • Using discretion and independent judgment in handling external complaints received documents and forwards to appropriate administrative staff.
  • Conducting thorough investigations of allegations of abuse, exploitation, abandonment and/or neglect
  • Conducting investigations in residences, personal care homes, long-term care facilities and nursing homes.  Requesting and reviewing medical records, collaborating with medical personnel regarding physical and mental capabilities of alleged victims, arranging for evaluations of capacity and securing financial records in cases of financial exploitation.
  • Developing care plans to eliminate or reduce risks of abuse, neglect, exploitation and/or abandonment and to meet persons' on-going medical and/or financial needs.
  • Documenting findings in an electronic database and use of standardized assessments, activity/journal notes and related entries within specified time frames. 
  • Responding both orally and in writing to allegations of abuse, neglect, exploitation and abandonment
  • Attending meetings to routinely discuss protective service cases, workload, work performance and concerns.
  • Works cooperatively within the office, Prairie Band Potawatomi Nation, and other agencies; as needed.
  • All other duties as assigned.

MINIMUM REQUIREMENTS:

  • Licensed Bachelor's Social Worker
  • One to two year abuse/neglect investigation experience
  • Licensed Social Worker
  • Must have a valid driver's license and be insurable with the Nation's insurance
  • Must be available to work the assigned schedule: Monday-Friday, 8:00am 4:30pm
  • Must possess the ability to maintain an effective working relationship with employee, vendors, Prairie Band Potawatomi Nation and other agencies.
  • Must possess professional work ethics, manner, and appearance.
  • Must possess ability to communicate professionally; both orally and in writing a service-orientated approach, sometimes in stressful circumstances.
  • Must be reliable, punctual and dependable in order to meet the needs of the department and Prairie Band Potawatomi Nation.
  • Must possess the ability to operate general office equipment, including but not limited to: 10-key calculator, telephone, computer and operating systems, computer software programs (Microsoft Office programs), fax/copier machine.
  • High level of interpersonal skills to handle sensitive and confidential situation and documentation.
  • Ability to maintain high level of confidentiality.
  • A record of satisfactory attendance and performance in all prior and current employment as evidenced by positive employment references from previous and current employers.

PREFERRED QUALIFICATIONS:

  • Licensed Masters Social Worker
  • Three to five years of abuse/neglect investigation experience
